Transient cerebral ischaemic attacks--management and prognosis

S Roden, T Low-Beer, M Carmalt

    Postgraduate Medical Journal
    |May 1, 1981

    Summary

Sulfinpyrazone did not reduce transient ischemic attack (TIA) recurrences in a short-term trial. However, avoiding early TIA recurrences significantly lowered the risk of future neurological events over two years.

    Area of Science:

    • Neurology
    • Clinical Pharmacology

    Background:

    • Transient ischemic attack (TIA) is a critical warning sign for future stroke.
    • Effective preventative strategies for TIA patients are crucial to reduce long-term neurological morbidity.

    Purpose of the Study:

    • To evaluate the efficacy of sulfinpyrazone in preventing recurrent transient ischemic attacks (TIAs).
    • To assess the long-term impact of early TIA recurrence on subsequent neurological events.

    Main Methods:

    • A double-blind, placebo-controlled, cross-over trial involving 50 patients with recent TIAs.
    • Patients received either sulfinpyrazone (200 mg 4 times daily) or placebo for 4-month periods.
    • Follow-up data were collected for up to 2 years on 39 patients.

    Main Results:

    Related Experiment Videos

    • No significant difference in TIA recurrence rates was observed between sulfinpyrazone and placebo groups during the 8-month trial.
    • A higher incidence of recurrences was noted in the initial 4 months of the study.
    • Long-term follow-up revealed that 90% of patients without early recurrences remained free of further neurological events, compared to only 47% of those who experienced recurrences.

    Conclusions:

    • Sulfinpyrazone did not demonstrate efficacy in preventing short-term TIA recurrences.
    • Absence of early TIA recurrence is a strong predictor of favorable long-term neurological outcomes.
    • Further research may be needed to identify effective treatments for TIA prevention.
