    Immune complexes (AgAb) are elevated in diabetics with severe microangiopathy, particularly when detected by the ClqSP method. This suggests a link between certain immune complexes and diabetic microvascular complications.

    Background:

    • Circulating immune complexes (AgAb) are implicated in various autoimmune and inflammatory conditions.
    • Diabetic microangiopathy, affecting small blood vessels, is a major complication of diabetes mellitus, leading to retinopathy and nephropathy.

    Purpose of the Study:

    • To investigate the prevalence and significance of circulating immune complexes (AgAb) in diabetic patients with and without severe microangiopathy.
    • To determine if different detection methods for AgAb correlate with microvascular complications in diabetes.

    Main Methods:

    • Studied 80 diabetics with severe microangiopathy and 71 without.
    • Utilized two distinct assays for AgAb detection: solid phase Clq binding test (ClqSP) and conglutinin binding test (KgBt).

    • The ClqSP-detectable AgAb are strongly associated with the presence of severe microangiopathy in diabetes.
    • Further research is warranted to elucidate the specific role of these heterogeneous AgAb in the pathogenesis of diabetic microvascular complications.
