    Summary

    Shigella enterocolitis can cause severe kidney failure in adults, a rare complication not previously documented. Prompt antibiotic treatment may aid recovery from this serious infection.

    Background:

    • Absence of sepsis, rhabdomyolysis, and HUS in this case highlights a unique presentation.
    • Treatment with trimethoprim-sulfamethoxazole was associated with the patient's recovery.

    • This case expands the known spectrum of renal complications associated with Shigella infections.
    • Clinicians should consider severe renal failure in the differential diagnosis of adult patients with Shigella enterocolitis.
    • Early recognition and appropriate antibiotic therapy are crucial for managing this rare but serious condition.
