Trazodone in depressed outpatients

K Rickels, W G Case

    The American Journal of Psychiatry
    |June 1, 1982
    PubMed
    Summary

Trazodone, a nontricyclic antidepressant, demonstrated efficacy comparable to amitriptyline in treating unipolar depression. Trazodone also showed a lower incidence of anticholinergic side effects compared to amitriptyline.

    Area of Science:

    • Psychiatry and Behavioral Sciences
    • Clinical Pharmacology

    Background:

    • Unipolar depression is a prevalent mood disorder requiring effective pharmacotherapy.
    • Tricyclic antidepressants like amitriptyline are established treatments but associated with side effects.
    • Nontricyclic antidepressants offer potential alternatives with improved tolerability profiles.

    Purpose of the Study:

    • To compare the clinical efficacy of trazodone (a nontricyclic antidepressant) with amitriptyline and placebo.
    • To evaluate the incidence of anticholinergic side effects associated with trazodone versus amitriptyline.

    Main Methods:

    • A double-blind study involving 202 outpatients diagnosed with unipolar depression.
    • Participants were randomized to receive trazodone, amitriptyline, or placebo.
    • Clinical improvement and side effect profiles were systematically assessed.

    Main Results:

    • Trazodone exhibited clinical efficacy similar to that of amitriptyline.
    • Both trazodone and amitriptyline demonstrated significantly greater clinical improvement compared to placebo.
    • Patients treated with trazodone experienced a lower incidence of anticholinergic side effects than those on amitriptyline.

    Conclusions:

    • Trazodone is an effective treatment option for unipolar depression, with efficacy comparable to amitriptyline.
    • Trazodone offers a favorable side effect profile, particularly regarding anticholinergic adverse events.
    • These findings support trazodone as a viable alternative for managing unipolar depression, especially in patients sensitive to anticholinergic effects.
