    Background:

    • Insulin-dependent diabetes (Type 1 diabetes) management is complex.
    • Understanding factors influencing glucose control is crucial for patient outcomes.

    Purpose of the Study:

    • To assess the impact of residual beta cell function, alpha cell function, and insulin binding antibodies on glucose control.
    • To determine the duration of residual beta cell function in patients with insulin-dependent diabetes.

    Main Methods:

    • Studied 44 patients with insulin-dependent diabetes of varying duration.
    • Measured residual beta cell secretion, plasma glucagon concentrations, and insulin binding antibodies.
    • Correlated these markers with indices of glucose control.

    Main Results:

    • Residual beta cell secretion was found in 40% of patients.
    • No residual function was observed in patients on insulin for over five years.
    • Better glucose control was significantly associated with residual beta cell secretion (p < 0.001).
    • No correlation was found between glucose control and glucagon levels or insulin antibodies.

    • Residual beta cell function plays a key role in maintaining better glucose control in the early years of insulin-dependent diabetes.
