    Background:

    • Postoperative bleeding is a significant complication following open-heart surgery.
    • Effective management strategies are crucial to minimize blood loss and avoid reoperation.

    Purpose of the Study:

    • To investigate the prophylactic and therapeutic efficacy of positive-end-expiratory pressure (PEEP) in controlling postoperative bleeding after open-heart operations.
    • To assess the safety and impact of PEEP on hemorrhage management.

    Main Methods:

    • One hundred open-heart surgery patients were studied.
    • Five cm of PEEP was applied post-sternal wire closure.
    • PEEP was incrementally increased for patients with bleeding exceeding 200 ml/hour.

    Main Results:

    • Total blood loss after 8 hours was 281 ml in the PEEP group versus 340 ml in controls (p > 0.05).
    • In patients with significant initial bleeding, PEEP application reduced bleeding to <60 ml/hour within 3 hours.
    • No PEEP-related complications or reoperations for hemorrhage were observed.

    Conclusions:

    • PEEP demonstrates a promising role in managing postoperative bleeding after open-heart surgery.
    • PEEP can be safely applied to reduce excessive hemorrhage and potentially avoid reoperation.
    • Further research may confirm PEEP as a standard adjunct in cardiac surgical recovery.
