    Background:

    • Microglia are traditionally classified as 'resting' or 'reactive' based on their appearance in brain tissue.
    • Previous understanding suggested a continuum between resting and reactive microglia states.

    Purpose of the Study:

    • To investigate the distinct origins and identities of 'resting' and 'reactive' microglia.
    • To clarify the cellular composition of the healthy central nervous system, particularly macrophages in cerebrospinal fluid (CSF).

    Main Methods:

    • Submicroscopic imaging
    • Cytokinetic analysis
    • Cytoimmunologic functional assays

    Main Results:

    • No relationship was found between 'resting' microglia and 'reactive' microglia.
    • 'Resting' microglia appear to be of neuro-ectodermal origin.
    • 'Reactive' microglia are derived from blood monocytes and are classified as brain macrophages. Macrophages are absent in intact brain parenchyma.
    • Cells in CSF spaces (subarachnoidal, perivascular, epiplexus, supraependymal) are mononuclear phagocytes and considered CSF macrophages.

    Conclusions:

    • 'Resting' microglia and 'reactive' microglia (brain macrophages) are distinct cell populations with different origins.
    • The healthy central nervous system contains an estimated maximum of 280,000 CSF macrophages.
