Aspirin and analgesic nephropathy

R D Emkey, J A Mills

    JAMA
    |January 1, 1982
    PubMed
    Summary

Long-term aspirin use, confirmed by blood tests, did not harm kidney function in rheumatoid arthritis patients. This study indicates that continuous aspirin ingestion is safe for renal health.

    Area of Science:

    • Nephrology
    • Rheumatology
    • Pharmacology

    Background:

    • Long-term aspirin use is common, particularly for patients with rheumatoid arthritis.
    • Concerns exist regarding potential adverse effects of chronic salicylate therapy on renal function.
    • Assessing the impact of prolonged aspirin ingestion on kidney health is crucial.

    Observation:

    • The study included 46 patients from Massachusetts General Hospital Arthritis Clinic with continuous aspirin use for over 10 years.
    • Aspirin ingestion was verified through random, unannounced blood salicylate level measurements.
    • The majority of participants had seropositive rheumatoid arthritis.

    Findings:

    • All patients maintained normal creatinine and blood urea nitrogen (BUN) levels.
    • Kidney concentrating ability, indicated by urine specific gravity, remained adequate in most patients (43/46 > 1.019).
    • These results suggest no significant renal impairment associated with long-term aspirin use.

    Implications:

    • Chronic aspirin therapy appears safe for renal function in patients with rheumatoid arthritis.
    • Findings may alleviate concerns about kidney damage from long-term salicylate use.
    • Further research could explore specific patient subgroups or alternative renal biomarkers.
