    Summary

    Tuberous sclerosis (TSC) patients rarely develop spontaneous pneumothorax, a serious complication. This study reviews 19 TSC cases, highlighting risks and management strategies for this rare pulmonary manifestation.

    Background:

    • Tuberous sclerosis (TSC) is a rare genetic disorder affecting multiple organs.
    • Pulmonary complications, including spontaneous pneumothorax, are infrequent but severe in TSC.
    • Adenoma sebaceum, intellectual disability, and epilepsy are characteristic TSC features.

    Observation:

    • Two new cases of spontaneous pneumothorax in TSC patients were identified.
    • A literature review identified 17 additional TSC patients with spontaneous pneumothorax.
    • Eight of the reviewed patients (42%) died due to pneumothorax.

    Findings:

    • Spontaneous pneumothorax is a significant risk in tuberous sclerosis patients.
    • Normal intelligence was observed in 53% of patients with pneumothorax, including all fatal cases.
    • Pathological and roentgenographic findings associated with TSC-related pneumothorax are detailed.

    Implications:

    • Early recognition and management of pulmonary complications are crucial in TSC.
    • Risk stratification for spontaneous pneumothorax in TSC patients is warranted.
    • Further research into the mechanisms and optimal treatment of TSC-associated lung disease is needed.
