Subtalar dislocation of the foot

J C DeLee, R Curtis

The Journal of Bone and Joint Surgery. American Volume
    |March 1, 1982

    Summary

Subtalar dislocations, especially lateral ones, often involve fractures and open injuries, leading to poor outcomes. Three weeks of cast immobilization and early motion exercises improve subtalar joint function.

    Area of Science:

    • Orthopedics
    • Traumatology
    • Podiatric Medicine

    Background:

    • Subtalar dislocations are rare but severe foot injuries.
    • Understanding associated injuries and their impact on outcomes is crucial for effective treatment.

    Purpose of the Study:

    • To analyze the characteristics and outcomes of subtalar dislocations.
    • To identify factors predicting poor results and recommend optimal management strategies.

    Main Methods:

    • Retrospective review of seventeen subtalar dislocation cases.
    • Classification of dislocations by direction (anterior, medial, lateral) and type (open, closed).
    • Assessment of associated fractures, treatment, and functional outcomes.

    Main Results:

    • Twelve medial, four lateral, and one anterior dislocation were identified.
    • Eight feet had associated talocalcaneal or talonavicular fractures; seven had ankle or metatarsal fractures.
    • Open dislocations and those requiring >3 weeks immobilization had poorer outcomes, particularly lateral dislocations.

    Conclusions:

    • Lateral subtalar dislocations have a higher risk of associated injuries and worse prognosis.
    • Optimal management involves three weeks of cast immobilization followed by early range-of-motion exercises for the subtalar joint.
