Background:
Paget's bone disease is a chronic disorder characterized by abnormal bone remodeling.
Secondary hyperparathyroidism (HPT) is a potential complication, but its exact role in Paget's disease requires further elucidation.
Purpose of the Study:
To investigate the prevalence and characteristics of parathormone (PTH) elevation in untreated Paget's bone disease.
To determine the association between high PTH levels and disease activity, calcium metabolism, and treatment response.
Main Methods:
Serum PTH levels were measured in 151 patients with untreated Paget's bone disease.
Calcium and urinary calcium levels were assessed.
Correlation analysis was performed to link PTH levels with disease spread, activity, and treatment outcomes.
Main Results:
A significant increase in serum PTH was found in 15% of patients.
Elevated PTH correlated with reduced serum calcium (calcemia) and urinary calcium (calciuria).
High PTH levels were not associated with disease spread or biological activity.
Conclusions:
The observed hyperparathyroidism in Paget's disease is secondary to increased calcium demands of pagetic bone.
This secondary HPT may contribute to the frequent occurrence of primary HPT in Paget's patients.
Secondary HPT could influence the efficacy of antiosteoclastic therapies like calcitonin and diphosphonates.
