    Summary

    Intracoronary streptokinase successfully restored blood flow in an occluded coronary artery in acute myocardial infarction patients. This rapid reperfusion relieved pain and improved cardiac function without complications.

    Background:

    • Acute myocardial infarction (AMI) requires rapid restoration of blood flow to minimize cardiac damage.
    • Intracoronary thrombolysis is an emerging treatment strategy for AMI.

    Observation:

    • A patient with an occluded right coronary artery experienced successful reperfusion with intracoronary streptokinase.
    • Vessel patency was achieved within 15 minutes of initiating the streptokinase infusion at 3000 U/min.

    Findings:

    • Successful reperfusion led to prompt relief of chest pain.
    • Electrocardiographic ST segments normalized post-reperfusion.
    • Significant improvements in cardiac output (3.6 to 6.3 L/min) and stroke volume (61 to 70 mL) were observed.
    • No adverse events occurred during the procedure.

    Implications:

    • Intracoronary thrombolysis with streptokinase demonstrates efficacy and safety in selected AMI patients.
    • This approach may become a preferred treatment for acute myocardial infarction.
    • Further research into intracoronary thrombolysis for AMI is warranted.
