    Summary

Choledochoduodenostomy is a safe and effective supplementary drainage procedure for post-cholecystectomy choledocholithiasis, especially in high-risk patients with biliary stasis. This surgical approach offers a definitive solution with low morbidity and no mortality.

    Area of Science:

    • Gastroenterology and Hepatobiliary Surgery
    • Surgical Techniques for Biliary Diseases

    Background:

    • Post-cholecystectomy choledocholithiasis presents challenges, particularly in patients with risk factors like prior surgery, advanced age, and prolonged symptoms.
    • Biliary stasis, indicated by bile infection and histological changes, is common in these high-risk patients.
    • Supplementary drainage procedures are considered for patients with biliary stasis undergoing choledochotomy.

    Purpose of the Study:

    • To evaluate the efficacy and safety of supplementary drainage procedures following choledochotomy in managing post-cholecystectomy choledocholithiasis.
    • To compare choledochoduodenostomy and sphincteroplasty as adjuncts to choledochotomy.

    Main Methods:

    • Assessment of 116 cases of post-cholecystectomy choledocholithiasis.
    • Implementation of choledochoduodenostomy and sphincteroplasty as supplementary drainage procedures to choledochotomy.
    • Evaluation of patient outcomes, including morbidity and mortality.

    Main Results:

    • Choledochoduodenostomy demonstrated a definitive solution for managing choledocholithiasis in the studied cohort.
    • The procedure was found to be safe and simple to perform.
    • No mortality was observed, and the morbidity rate was very low.

    Conclusions:

    • Choledochoduodenostomy is a highly effective and safe supplementary procedure for post-cholecystectomy choledocholithiasis, particularly in patients with risk factors for biliary stasis.
    • The technique offers a definitive and low-risk solution, making it a valuable option in hepatobiliary surgery.
