    Summary

    Airway hyperreactivity is not always present in asthma patients. Some asthmatics show minimal response to acetylcholine, even during an asthma attack, challenging the constant presence of this feature.

    Background:

    • Bronchial asthma is often characterized by airway hyperreactivity.
    • The consistency of airway hyperreactivity in asthma patients requires further investigation.

    Purpose of the Study:

    • To investigate the variability of airway hyperreactivity in patients with bronchial asthma.
    • To determine if airway hyperreactivity is a constant feature across different asthma phenotypes and conditions.

    Main Methods:

    • Assessed changes in forced expiratory volume in 1 second (FEV1) after acetylcholine inhalation in asthmatic patients.
    • Studied patients before, during, and after asthmatic episodes.
    • Compared responses in intrinsic asthma, extrinsic asthma, and healthy subjects.

    Main Results:

    • Nine asthmatic patients showed less than 10% decrease in FEV1 after acetylcholine.
    • Four intrinsic asthma patients had significantly lower FEV1 during obstruction (P<0.02).
    • Average FEV1 decrease post-acetylcholine was 5% before/after obstruction, and 4.7% post-histamine; healthy subjects averaged <3%, while 14 asthmatics averaged 40%.

    Conclusions:

    • Airway hyperreactivity is not a constant feature in all bronchial asthma patients.
    • Asthma severity and phenotype may influence the presence and degree of airway hyperreactivity.
    • Findings suggest a need for personalized assessment of airway hyperreactivity in asthma management.
