Hand involvement in psoriatic arthritis

M R Belsky, P Feldon, L H Millender

    The Journal of Hand Surgery
    |March 1, 1982
    PubMed
    Summary

Psoriatic arthritis hand surgery patients showed unique joint involvement and surgical outcomes distinct from rheumatoid disease. Findings include spontaneous fusions and increased infection risk after arthroplasty.

    Area of Science:

    • Orthopedics
    • Rheumatology
    • Hand Surgery

    Background:

    • Psoriatic arthritis (PsA) is a chronic inflammatory disease.
    • Hand and wrist involvement is common in PsA, often leading to significant disability.
    • Surgical management in PsA hands differs from other inflammatory arthropathies.

    Purpose of the Study:

    • To retrospectively evaluate clinical and radiographic patterns of hand and wrist involvement in PsA patients requiring surgery.
    • To assess surgical outcomes in PsA patients and compare them to rheumatoid disease.
    • To identify characteristic features of PsA hand and wrist disease.

    Main Methods:

    • Retrospective review of 25 PsA patients who underwent hand surgery.
    • Clinical and radiographic evaluation of joint involvement and surgical results.
    • Comparison of PsA hand/wrist patterns and surgical outcomes with rheumatoid disease.

    Main Results:

    • PsA hand surgery patients exhibited distinct patterns: spontaneous wrist fusion, severe proximal interphalangeal (PIP) joint involvement with flexion contractures, distal interphalangeal (DIP) joint erosion with fusion, and generalized stiffness.
    • Surgical intervention yielded minimal improvement in motion.
    • Arthroplasty in PsA patients was associated with a significantly increased incidence of infection.

    Conclusions:

    • Hand and wrist involvement in PsA presents unique challenges and surgical considerations compared to rheumatoid arthritis.
    • Surgical outcomes for PsA hands are often limited, with a notable risk of postoperative infection.
    • Further research is needed to optimize surgical strategies for PsA patients.
