Fatal pneumothorax in "BCNU lung"

K S Wilson, M L Brigden, S Alexander

    Medical and Pediatric Oncology
    |January 1, 1982

    Summary

Carmustine (BCNU) treatment for recurrent glioma can cause severe lung damage, including pneumothorax and pulmonary fibrosis. This "BCNU lung" can be fatal, even when the cancer is in remission.

    Area of Science:

    • Oncology
    • Pulmonology
    • Toxicology

    Background:

    • Recurrent gliomas pose significant treatment challenges.
    • Carmustine (BCNU) is a chemotherapy agent used in glioma treatment.

    Observation:

    • A patient with recurrent glioma received BCNU treatment.
    • The patient subsequently developed pneumothorax and interstitial pulmonary fibrosis.

    Findings:

    • The patient's lung complications were attributed to BCNU toxicity, termed "BCNU lung".
    • These pulmonary issues led to the patient's death, despite successful remission of the glioma.

    Implications:

    • Highlights the critical need for vigilant pulmonary monitoring in patients undergoing BCNU therapy.
    • Underscores the potential for severe, life-threatening chemotherapy-induced lung disease.
    • Suggests a need for risk-benefit assessment and potential alternative therapies for BCNU-treated patients.
