Carcinoid tumors: a 37-year experience

J Zeitels, K Naunheim, E L Kaplan

Archives of Surgery (Chicago, Ill. : 1960)

    Summary

Carcinoid tumors, particularly small rectal and ileal ones, generally have a favorable prognosis. Larger tumors (>2 cm) and those in the ileum or colon show higher metastasis rates, impacting survival.

    Area of Science:

    • Gastroenterology
    • Oncology
    • Surgical Pathology

    Background:

    • Carcinoid tumors are neuroendocrine neoplasms with variable clinical behavior.
    • Understanding the prognostic factors influencing carcinoid tumor outcomes is crucial for patient management.

    Purpose of the Study:

    • To analyze the clinical characteristics and prognostic factors of carcinoid tumors.
    • To evaluate the relationship between tumor size, location, and metastasis in carcinoid tumors.

    Main Methods:

    • Retrospective analysis of 101 carcinoid tumor cases.
    • Review of patient data from University of Chicago hospitals (1942-1979).
    • Correlation of tumor characteristics (size, location) with metastasis and survival.

    Main Results:

    • Rectum (29%) and ileum (28%) were the most common sites; appendiceal tumors were 12%.
    • Smaller tumors (<1 cm, 60%) and those confined to the submucosa (43%) had better prognoses.
    • Tumors >2 cm had a 70% metastasis rate, versus 6% for <1 cm lesions. Ileal (46%) and colonic (40%) lesions showed higher metastasis rates than bronchial or appendiceal.
    • Overall five-year survival was 87%, significantly lower with distant spread (40%).

    Conclusions:

    • Tumor size and location are critical prognostic indicators for carcinoid tumors.
    • Early detection and management of smaller, localized carcinoid tumors improve patient survival.
    • Further research into specific tumor site behavior and metastasis patterns is warranted.
