    Summary

A 71-year-old woman developed severe thrombocytopenia after starting ethambutol treatment. Her condition improved after stopping the drug, suggesting a new side effect of ethambutol.

    Area of Science:

    • Pharmacology
    • Hematology
    • Immunology

    Background:

    • Ethambutol is a first-line medication for tuberculosis treatment.
    • Adverse drug reactions can significantly impact patient management and treatment outcomes.

    Observation:

    • A 71-year-old female patient presented with severe thrombocytopenia, purpura, and melena.
    • These symptoms emerged four days after initiating ethambutol therapy.

    Findings:

    • Macrophage inhibition factor testing confirmed ethambutol as the causative agent.
    • Thrombocytopenia resolved rapidly upon cessation of ethambutol treatment.

    Implications:

    • This case suggests thrombocytopenia is a previously unreported adverse effect of ethambutol.
    • Clinicians should consider ethambutol-induced thrombocytopenia in patients presenting with compatible symptoms.
    • Further investigation into the immunologic mechanisms of ethambutol-related hematologic toxicity is warranted.
