    Summary

    This study investigated catecholamines in pregnancy, finding higher fetal levels than maternal. Placental metabolism and significant individual variations in the fetal sympathoadrenal system were observed.

    Background:

    • The maternal and fetal sympathoadrenal system plays a crucial role during pregnancy.
    • Understanding catecholamine (CA) levels and placental function is vital for perinatal health.

    Purpose of the Study:

    • To investigate the release of norepinephrine (NA) and epinephrine (A) in the maternal and fetal sympathoadrenal systems.
    • To assess the impact of anesthesia induction on CA levels.
    • To evaluate placental metabolism of CAs and fetal sympathoadrenal activity.

    Main Methods:

    • Radioenzymatic measurement of NA and A in 20 women undergoing cesarean section.
    • Atropinization administered before anesthesia induction.
    • Comparison of maternal plasma CA levels with fetal umbilical artery and vein CA concentrations.

    Main Results:

    • Maternal plasma NA and A levels showed no significant changes related to anesthesia induction.
    • Fetal umbilical arterial NA and A concentrations were significantly higher than maternal values.
    • Umbilical artery CA levels were 3-6 times higher than umbilical vein levels, suggesting placental metabolism.
    • High individual variation in fetal NA and A levels indicated altered fetal sympathoadrenal system activity.

    Conclusions:

    • Placental metabolism significantly impacts catecholamine concentrations.
    • The fetal sympathoadrenal system exhibits substantial individual variability during late-term pregnancy.
    • Anesthesia induction did not significantly alter maternal catecholamine levels in this cohort.
