Improved immunization should reduce bronchiectasis, but ciliary defects remain a cause. This study identified anatomical and physiological abnormalities in the cilia of patients with bronchiectasis.

    Area of Science:

    • Pulmonology
    • Cell Biology
    • Medical Diagnostics

    Background:

    • Bronchiectasis incidence is expected to decline due to widespread immunization programs.
    • However, underlying etiological factors, such as ciliary dysfunction, may still contribute to bronchiectasis development.

    Observation:

    • Fifteen patients with bronchiectasis underwent examination for ciliary defects.
    • Methods included electron microscopy of nasal mucosa and lung clearance assessment using technetium-99m and a gamma camera.

    Findings:

    • Three patients exhibited significant ciliary defects, including compound cilia, structural disorganization, vacuolization, and deficient inner dynein arms.
    • One patient transiently displayed a loss of cell membranes, which resolved over four months.

    Related Experiment Videos

    Implications:

    • Ciliary abnormalities, whether anatomical or physiological, are implicated as a cause of bronchiectasis.
    • 'Sick cilia' syndrome should be considered in the differential diagnosis of bronchiectasis, even with improved vaccination rates.
