    Summary

This case study highlights a rare combination of traumatic pseudoaneurysm and arteriovenous fistula in the middle meningeal artery. Traumatic pseudoaneurysms often present with delayed hemorrhage, explaining prolonged lucid intervals in some patients.

    Area of Science:

    • Neurology
    • Neurosurgery
    • Radiology

    Background:

    • Subarachnoid hemorrhage (SAH) symptoms can mask other underlying pathologies.
    • Traumatic cerebrovascular injuries require careful diagnosis and management.

    Observation:

    • A patient presented with SAH symptoms but was diagnosed with a rare combination of traumatic pseudoaneurysm and arteriovenous fistula of the middle meningeal artery.
    • Literature review indicated distinct clinical courses for traumatic pseudoaneurysms versus arteriovenous fistulas.

    Findings:

    • Traumatic pseudoaneurysms of the middle meningeal artery are frequently associated with delayed hemorrhage.
    • This delayed bleeding mechanism may explain the prolonged lucid intervals observed in some patients with epidural hematomas.

    Implications:

    • Understanding these distinct injury patterns is crucial for accurate diagnosis and timely intervention.
    • Recognizing the potential for delayed hemorrhage in traumatic pseudoaneurysms can improve patient outcomes and management strategies.
