    Summary

    Sympathetic ophthalmia, a rare eye condition, can occur after trauma or surgery. Vitrectomy surgery alone presents a low risk, similar to other uveal tract procedures, but risk increases with complications.

    Background:

    • Sympathetic ophthalmia is a rare but serious bilateral intraocular inflammation.
    • It can occur following ocular trauma or surgery.

    Purpose of the Study:

    • To assess the incidence of sympathetic ophthalmia after vitrectomy.
    • To compare the risk of sympathetic ophthalmia after vitrectomy with other ocular procedures.

    Main Methods:

    • Survey of eye pathology laboratories and retinal surgeons.
    • Analysis of case data for sympathetic ophthalmia incidence.
    • Calculation of risk based on surgical procedures, including vitrectomy.

    Main Results:

    • Sympathetic ophthalmia occurred in 2 per 1,000 eyes examined in a laboratory survey.
    • Of 53 cases, 55% were posttraumatic and 45% postsurgical.
    • Vitrectomy was involved in 19% of sympathetic ophthalmia cases; risk was 0.06% after 14,915 vitrectomies.
    • Vitrectomy alone in an uninjured eye had a 0.01% incidence of sympathetic ophthalmia.

    Conclusions:

    • The risk of sympathetic ophthalmia after vitrectomy alone is low and comparable to other uveal tract penetrating procedures.
    • Risk increases with combined accidental trauma or additional surgical interventions.
