    Summary

    Sulfinpyrazone can cause kidney failure, even in patients with existing renal issues. Monitor kidney function closely and stop sulfinpyrazone if blood urea nitrogen or creatinine levels rise.

    Background:

    • Sulfinpyrazone is a uricosuric agent used to treat gout.
    • Renal complications associated with sulfinpyrazone are not well-documented.

    Observation:

    • Three patients developed acute renal failure shortly after initiating sulfinpyrazone therapy.
    • One patient experienced reversible renal dysfunction, while another died from myocardial infarction.

    Findings:

    • The clinical presentation of renal failure was atypical.
    • Elevated creatinine levels were noted in two patients prior to sulfinpyrazone administration.
    • A third patient developed renal failure and subsequently died from myocardial infarction.

    Implications:

    • Close monitoring of renal function (blood urea nitrogen and creatinine) is recommended 2-4 days after starting sulfinpyrazone.
