    Summary

Multiple cholesterol emboli syndrome (MCES) can cause acute renal failure and distinctive skin lesions. Penile and scrotal necrosis may be previously unrecognized indicators of MCES.

    Area of Science:

    • Nephrology
    • Dermatology
    • Vascular Medicine

    Background:

    • Multiple cholesterol emboli syndrome (MCES) is a serious condition often triggered by invasive vascular procedures.
    • MCES can lead to significant organ damage, particularly affecting the kidneys.

    Purpose of the Study:

    • To report on four cases of MCES, focusing on renal and cutaneous manifestations.
    • To identify potential new clinical clues for the early diagnosis of MCES.

    Main Methods:

    • Case series analysis of four patients diagnosed with MCES.
    • Clinical and procedural history review, including radiocontrast studies.
    • Detailed examination of dermatological findings, including "purple toe" syndrome and genital necrosis.

    Main Results:

    • Three of four patients presented with acute renal failure; one achieved stable renal function.
    • All patients exhibited the characteristic "purple toe" livido reticularis.
    • Three patients had previously unreported penile and scrotal necrosis.

    Conclusions:

    • Acute renal failure is a common complication of MCES.
    • Penile and scrotal necrosis may serve as additional diagnostic indicators for MCES.
    • Early recognition of these signs can aid in timely diagnosis and management of MCES.
