    Background:

    • Best's vitelliform macular dystrophy is an inherited condition affecting central vision.
    • Pseudohypopyon, or fluid in the anterior chamber, is an unusual finding in this condition.

    Observation:

    • Ten eyes across seven patients with Best's disease and pseudohypopyon were analyzed.
    • Fluorescein angiography revealed hyperfluorescent defects in the superior retinal pigment epithelium.
    • Electro-oculography was abnormal in all tested patients.

    Findings:

    • Fluid in the pseudohypopyon shifted slightly with positional changes, suggesting it resides between the retinal pigment epithelium and sensory retina.
    • The volume of the hypopyon increased in one observed eye.
    • Abnormal electro-oculography results correlate with disease severity.

    Implications:

    • These findings enhance understanding of the anatomical location of fluid in Best's disease.
    • This may guide future diagnostic and therapeutic strategies for managing pseudohypopyon in macular dystrophies.
    • Further research is warranted to explore the pathogenesis and clinical significance of pseudohypopyon in Best's vitelliform macular dystrophy.
