Retinal phlebitis with chorioretinal emboli

    Summary

    Ocular embolism can lead to retinal vascular occlusive disease and retinal periphlebitis. This study found chorioretinal calcific emboli in patients with systemic thromboembolic disease, suggesting a link to retinal phlebitis.

    Background:

    • Emboli in the eye can cause retinal vascular occlusive disease, presenting diverse clinicopathologic features.
    • Systemic embolic disease has been linked to progressive obliterative arteriolitis, raising questions about retinal vasculitis as a potential feature.

    Observation:

    • Postmortem examination of two elderly female patients revealed retinal periphlebitis and chorioretinal calcific emboli.
    • Both patients exhibited systemic thromboembolic disease originating from the heart and great vessels.

    Findings:

    • Histopathologic analysis confirmed retinal periphlebitis associated with numerous calcific emboli in the choroid and retina.
    • One patient presented with decreased visual acuity, paracentral scotoma, and perivascular sheathing, indicative of ocular involvement.

    Implications:

    • These findings suggest that ocular embolism may be a contributing factor in the development of retinal phlebitis.
    • The study highlights a potential link between systemic thromboembolic events and specific intraocular inflammatory conditions.
