    Summary

    Observation:

    • A case of a large suprarenal gland pseudocyst (20x10x10 cm) containing 2 liters of fluid is presented.
    • The cyst caused significant mass effect, pushing the right kidney into the left retroperitoneum, resulting in crossed ectopia.
    • Imaging revealed the cyst's mass effect and the anomalous course of the right renal artery crossing the aorta.

    Findings:

    • Intravenous urography (IVU) demonstrated right kidney crossed ectopia and the suprarenal mass.
    • Angiography detailed the right renal artery's unusual medial course across the aorta to supply the displaced kidney.
    • Surgical intervention confirmed the diagnosis, involving cyst removal and right nephropexy.

    Implications:

    • This case highlights the diagnostic challenges and anatomical variations associated with large suprarenal cysts.
    • Successful surgical management, including cyst resection and nephropexy, can restore normal anatomy and function.
    • Further research into the classification, pathohistology, and pathogenesis of these rare cysts is warranted.
