    A patient with multiple myeloma (MM) developed Kaposi sarcoma (KS) potentially linked to melphalan treatment. Discontinuing melphalan halted KS spread, but restarting it caused advancement, suggesting a treatment connection.

    Background:

    • Multiple myeloma (MM) is a cancer of plasma cells.
    • Kaposi sarcoma (KS) is a cancer that develops from the cells that line lymph or blood vessels.

    Observation:

    • A patient diagnosed with MM developed KS 18 months later.
    • KS appeared shortly after initiating melphalan therapy.
    • KS progression was halted by discontinuing melphalan and using irradiation and bleomycin.

    Findings:

    • Resuming melphalan treatment led to KS advancement.
    • A potential association between melphalan and KS development in MM patients is suggested.

    • This case highlights a possible adverse effect of melphalan in MM patients.
    • Further investigation into the link between MM treatments and secondary malignancies like KS is warranted.
