Postural hypotension and diuretic therapy in the elderly

M G Myers, P M Kearns, D S Kennedy

    Canadian Medical Association Journal
    |September 23, 1978

    Summary

This study found that while systolic blood pressure peaked in older adults, diastolic pressure declined with age. Postural hypotension was infrequent and not significantly linked to aging in this elderly cohort.

    Area of Science:

    • Gerontology
    • Cardiovascular Medicine
    • Clinical Physiology

    Background:

    • Age-related changes in blood pressure regulation are complex.
    • Understanding blood pressure trends and postural changes in the elderly is crucial for clinical management.

    Purpose of the Study:

    • To investigate blood pressure patterns, including systolic and diastolic pressures, in ambulatory individuals aged 50 to 99 years.
    • To assess the prevalence and characteristics of postural hypotension across different age decades in this population.

    Main Methods:

    • Ambulatory blood pressure monitoring was conducted in 319 subjects (predominantly men) aged 50-99 years.
    • Systolic and diastolic blood pressures were analyzed in supine and erect positions.
    • The frequency of postural hypotension was calculated and compared between age groups and diuretic use.

    Main Results:

    • Mean systolic blood pressure peaked in the seventh and eighth decades.
    • Mean diastolic blood pressure showed a progressive decline after age 69.
    • The prevalence of significant postural hypotension (≥20 mm Hg drop) was low and similar across age groups; diuretic use showed a slightly higher frequency.

    Conclusions:

    • Blood pressure does not uniformly increase with age in this elderly population.
    • Postural hypotension is not a consistent finding in aging and appears infrequent.
    • Further research may clarify the role of medications like diuretics in postural blood pressure changes.
