Mediastinal rhabdomyoma

    Summary

    This is the first reported case of a mediastinal rhabdomyoma in an 80-year-old male. Ultrastructural analysis suggests an extra-cardiac origin, likely from thymic myoid cells.

    Background:

    • Mediastinal tumors are rare, with rhabdomyomas typically found in the heart.
    • This case presents a unique instance of a non-cardiac rhabdomyoma.

    Observation:

    • An 80-year-old male presented with gastric adenocarcinoma, liver metastases, and parathyroid adenomas.
    • A distinct rhabdomyoma was identified in the anterior-superior mediastinum.

    Findings:

    • Ultrastructural examination revealed haphazardly arranged myofilaments with prominent Z bands.
    • The absence of desmosomes indicated an extra-cardiac, rather than cardiac, origin.
    • Histological features suggest a probable origin from thymic myoid cells.

    Implications:

    • This finding expands the known locations for rhabdomyomas.
    • It highlights the importance of considering thymic myoid cells in the differential diagnosis of mediastinal tumors.
    • Further research into the tumorigenesis of mediastinal rhabdomyomas is warranted.
