    Summary

    A traumatic fall led to acquired os odontoideum in a child, causing significant atlantoaxial instability. Surgical fusion (atlantoaxial arthrodesis) effectively treated this rare condition.

    Background:

    • Acquired os odontoideum is a rare condition.
    • It is often associated with spinal cord injury.
    • Trauma is a potential cause.

    Observation:

    • A 7-year-old boy presented with symptoms of os odontoideum.
    • The condition developed after a fall at 19 months of age.
    • Flexion-extension films showed marked atlantoaxial instability.

    Findings:

    • The patient had acquired os odontoideum.
    • This rare entity is hypothesized to result from trauma-induced vascular insufficiency.
    • Atlantoaxial instability is a common complication.

    Implications:

    • Acquired os odontoideum can lead to severe spinal cord compromise.
    • Atlantoaxial instability in os odontoideum is an indication for surgical intervention.
    • Atlantoaxial arthrodesis is an effective treatment for this condition.
