    Observation:

    • A 15-year-old patient developed bilateral optic atrophy during treatment for medulloblastoma.
    • The patient received concurrent vincristine sulfate and neuraxis radiation therapy.
    • Other neurological symptoms like peripheral neuropathy and confusion were noted.

    Findings:

    • No increased intracranial pressure or active tumor was detected.
    • Discontinuation of vincristine sulfate led to the reversal of visual loss and other neurotoxicities.
    • Optic atrophy in this context was likely drug-induced, potentially exacerbated by radiation.

    Implications:

    • Physicians should consider vincristine-induced optic neuropathy in patients presenting with visual loss.
    • Concomitant or prior cranial radiation may increase the risk of vincristine neurotoxicity.
    • Prompt discontinuation of vincristine is recommended for potential visual recovery.
