    Background:

    • Transcortical sensory aphasia (TSA) is a language disorder.
    • It is characterized by impaired auditory comprehension and fluent speech production.
    • Repetition abilities are typically preserved in TSA.

    Purpose of the Study:

    • To identify the precise anatomical location of brain lesions in patients with TSA.
    • To correlate lesion characteristics with clinical presentation and prognosis.

    Main Methods:

    • Computed tomography (CT) and isotope scans were used to visualize brain lesions in 15 patients.
    • An overlap technique was employed to determine the common lesion location.
    • Correlation analysis was performed between lesion size and clinical severity.

    Main Results:

    • Lesions were consistently located in the posterior parieto-occipital region.
    • Two distinct lesion groups were identified: one in the posterior cerebral artery territory and another in a watershed area.
    • Lesion size correlated significantly with the severity of aphasia.

    Conclusions:

    • The posterior parieto-occipital region is critical for transcortical sensory aphasia.
    • Lesions in specific vascular territories (PCA and watershed areas) are implicated.
    • TSA is associated with a generally good prognosis and frequent recovery, sometimes with transient visual deficits.
