    Summary

    High viscosity liquid silicone injection successfully reattached retinas in 85% of complicated retinal detachment cases. Vision improved, and silicone toxicity concerns appear overstated.

    Background:

    • Complicated retinal detachments pose significant surgical challenges.
    • Previous concerns existed regarding the toxicity of liquid silicone in ocular applications.

    Purpose of the Study:

    • To evaluate the efficacy of high viscosity liquid silicone injection for treating complicated retinal detachments.
    • To assess visual outcomes and anatomical reattachment rates.
    • To address potential silicone toxicity concerns.

    Main Methods:

    • Liquid silicone (12500 centistokes) was injected into the vitreous of 47 eyes with complicated retinal detachments.
    • Eyes were considered inoperable by conventional scleral buckling and/or vitrectomy techniques.
    • Follow-up ranged from 3 months to 2 years.

    Main Results:

    • Complete anatomical reattachment was achieved in 72% (34/47) of eyes.
    • Partial reattachment was achieved in 13% (6/47) of eyes.
    • All successfully reattached eyes showed visual improvement, typically to count fingers or 6/60.

    Conclusions:

    • High viscosity liquid silicone injection is an effective treatment for selected cases of complicated retinal detachment.
    • The procedure offers a viable option for eyes deemed inoperable by other methods.
    • Evidence suggests liquid silicone toxicity may be less significant than previously reported.
