Post-thyroidectomy, while thyroid hormones and calcitonin may leak, calcitonin is unlikely to cause hypocalcemia. Impaired parathyroid hormone secretion may contribute to low calcium levels after thyroid surgery.

    Area of Science:

    • Endocrinology
    • Surgical Physiology

    Background:

    • Thyroidectomy is a common surgical procedure for hyperthyroidism.
    • Postoperative hypocalcemia is a potential complication following thyroidectomy.
    • The roles of calcitonin and parathyroid hormone in this complication require further elucidation.

    Purpose of the Study:

    • To investigate the dynamic changes in plasma calcitonin, parathyroid hormone, thyroid hormones, and calcium levels in patients undergoing thyroidectomy for hyperthyroidism.
    • To assess the potential contribution of calcitonin and parathyroid hormone to the development of postoperative hypocalcemia.

    Main Methods:

    • Blood samples were collected from 6 patients before, during, and after thyroidectomy.
    • Plasma levels of calcitonin, parathyroid hormone, total thyroid hormones, and calcium were measured.

    Related Experiment Videos

  • Changes in hormone levels and calcium were correlated with the surgical procedure.
  • Main Results:

    • A significant decrease in plasma calcium was observed postoperatively in 4 out of 6 patients.
    • No significant changes in calcitonin or parathyroid hormone levels were noted in these 4 patients.
    • In one patient with a pronounced calcium drop, transient increases in thyroid hormones and calcitonin were observed, but calcitonin levels rose after the initial decrease in calcium.

    Conclusions:

    • Leakage of thyroid hormones and calcitonin from the thyroid remnant post-surgery is possible.
    • Calcitonin is unlikely to be a primary factor in the development of postoperative hypocalcemia.
    • Impaired parathyroid hormone secretion may be a contributing factor to postoperative hypocalcemia after thyroidectomy.
