    Background:

    • Synovial chondromatosis is a benign neoplastic condition characterized by the formation of cartilaginous nodules within the synovial membrane.

    Observation:

    • Electron microscopy was utilized to examine three cases of knee synovial chondromatosis.
    • Hyaline cartilage nodules exhibited ultrastructural characteristics of mature chondrocytes.
    • Adjacent areas showed numerous fibroblast-like cells embedded in a matrix with abundant coarse cartilage.

    Findings:

    • Intermediate cells with features of both chondrocytes and fibroblasts were observed, possessing well-developed Golgi complexes.
    • These intermediate cells suggest active metaplasia, where fibrocytes differentiate into chondrocytes.
    • The findings indicate a dynamic process of chondroid matrix secretion at the interface between fibrous tissue and cartilage nodules.

    Implications:

    • This study provides ultrastructural evidence supporting active metaplasia in synovial chondromatosis pathogenesis.
    • Identifying the sites of active metaplasia may offer new targets for therapeutic interventions.
