Ibuprofen in canine endotoxin shock

E R Jacobs, M E Soulsby, R C Bone

    The Journal of Clinical Investigation

    Summary

Ibuprofen, a cyclooxygenase inhibitor, reversed dangerous drops in blood pressure and improved cardiac function in canine endotoxin shock. This study demonstrates ibuprofen

    Area of Science:

    • Pharmacology
    • Physiology
    • Critical Care Medicine

    Background:

    • Prostaglandins play a key role in endotoxin shock pathophysiology.
    • Prostaglandin synthetase inhibitors are established tools for studying these effects.

    Purpose of the Study:

    • To investigate ibuprofen's efficacy in reversing hemodynamic and acid-base disturbances in canine endotoxin shock.
    • To evaluate ibuprofen as a specific cyclooxygenase inhibitor in this model.

    Main Methods:

    • Induction of endotoxin shock in canines.
    • Administration of ibuprofen, a cyclooxygenase inhibitor, post-endotoxin.
    • Monitoring of systemic blood pressure, cardiac index, and arterial pH.

    Main Results:

  • Ibuprofen administration significantly improved mean blood pressure compared to endotoxin alone (150.2 vs. <83 mm Hg).
  • Cardiac index was markedly increased in ibuprofen-treated dogs (2.3 vs. 1.0 liter/m2 per min).
  • Ibuprofen prevented the development of acidosis, maintaining arterial pH (7.36 vs. 7.18).
  • Conclusions:

    • Ibuprofen effectively protects against hypotension in canine endotoxin shock.
    • Ibuprofen mitigates the depression of cardiac index associated with endotoxin shock.
    • Ibuprofen prevents the acid-base disturbances, specifically acidosis, in this model.
