    Background:

    • Hemifacial spasm management often involves retromastoid craniectomy.
    • Intraoperative monitoring of brain stem auditory evoked potentials (BAEPs) is crucial for assessing auditory pathway integrity.

    Observation:

    • A patient undergoing retromastoid craniectomy for hemifacial spasm exhibited reversible changes in BAEPs.
    • These alterations included increased peak latencies and decreased amplitudes.

    Findings:

    • BAEP changes persisted for up to 3 hours while the head was in pin fixation.
    • Auditory function normalized immediately upon head repositioning to a neutral neck posture.
    • Similar BAEP alterations were observed during a second surgical procedure.
