Creatine kinase and recurrent myocardial infarction

R Löfmark

    Summary

This study monitored creatine kinase (CK) levels in 31 acute myocardial infarction (MI) patients. Findings suggest reinfarction may not be a stepwise process, as no elevated CK levels were observed in patients experiencing recurrent MI during hospitalization.

    Area of Science:

    • Cardiology
    • Biochemistry

    Background:

    • Acute myocardial infarction (MI) is a leading cause of mortality.
    • Understanding reinfarction patterns is crucial for patient management.

    Purpose of the Study:

    • To investigate the role of daily serum creatine kinase (CK) measurements in diagnosing silent or recurrent myocardial infarctions during hospitalization.
    • To evaluate the hypothesis that reinfarction occurs as a stepwise myocardial damage process.

    Main Methods:

    • Daily serum creatine kinase (CK) activity was measured in 31 patients hospitalized for acute MI.
    • CK isoenzyme B was analyzed when CK levels were elevated.
    • Patient data was monitored for new MI diagnoses and recurrences within three months post-discharge.

    Main Results:

    • One probable silent MI and two overt MIs were diagnosed during hospitalization based on CK levels.
    • Five patients experienced MI recurrence within three months after discharge.
    • None of the eight patients diagnosed with a new or recurrent MI during or shortly after hospitalization showed elevated CK levels while hospitalized.

    Conclusions:

    • Daily CK monitoring during hospitalization can aid in diagnosing silent or overt MIs.
    • The study's findings do not support the theory of stepwise myocardial damage in reinfarction.
    • Further research is needed to elucidate the mechanisms of reinfarction.
