Mixed meal tolerance test and reactive hypoglycemia

R W Buss, P C Kansal, R F Roddam

    Summary

Reactive hypoglycemia is uncommon when diagnosed with a mixed meal tolerance test (MMTT) instead of an oral glucose tolerance test (OGTT). MMTT better reflects natural eating patterns, leading to different results in patients with hypoglycemia symptoms.

    Area of Science:

    • Endocrinology
    • Metabolic Disorders
    • Gastroenterology

    Background:

    • Postprandial hypoglycemia presents with symptoms after meals.
    • Diagnosis often relies on oral glucose tolerance tests (OGTT).

    Purpose of the Study:

    • To compare the diagnostic utility of OGTT versus mixed meal tolerance tests (MMTT) in patients with suspected reactive hypoglycemia.

    Main Methods:

    • Twenty-six patients with hypoglycemia symptoms underwent OGTT.
    • Ten patients with symptomatic hypoglycemia during OGTT then underwent MMTT.

    Main Results:

    • Symptomatic hypoglycemia occurred in 38.5% of patients during OGTT.
    • No symptomatic hypoglycemia was observed in any patient during MMTT.

    Related Experiment Videos

  • Nadir glucose levels were significantly lower during OGTT compared to MMTT (44.1 vs. 77.3 mg/dl).
  • Insulin peaked earlier during MMTT than OGTT (46.7 vs. 86.7 minutes).
  • Conclusions:

    • Reactive hypoglycemia is uncommon when diagnosed using a more natural stimulus like MMTT.
    • The timing of insulin secretion, which is earlier during MMTT, may explain the lack of symptomatic hypoglycemia.
