    Summary

    Periodic electroencephalogram (EEG) complexes in viral encephalitis typically suggest herpes simplex virus. However, this case shows infectious mononucleosis can also cause these EEG patterns, expanding the differential diagnosis for encephalitis.

    Background:

    • Periodic electroencephalogram (EEG) complexes are often considered indicative of herpes simplex virus (HSV) encephalitis.
    • Acute viral encephalitis presents with diverse neurological symptoms and requires accurate diagnosis for appropriate treatment.

    Observation:

    • A patient with infectious mononucleosis exhibited severe neurological manifestations, including lymphocyte meningitis, coma, seizures, aphasia, hemiparesis, and hemianopsia.
    • Serial EEGs revealed periodic, left-sided slow wave complexes occurring every 4-5 seconds, consistent with those seen in HSV encephalitis.

    Findings:

    • The periodic EEG complexes in this patient resolved with clinical improvement, correlating with the resolution of infectious mononucleosis.
    • This case, along with previous reports, demonstrates that periodic EEG complexes are not pathognomonic for HSV encephalitis.
