Transvesical approach to utricular cysts

    Summary

    Utricular cysts, often seen in intersex conditions, can cause severe infections and incontinence. A transvesical surgical approach is recommended for better outcomes compared to the perineal method.

    Background:

    • Utricular cysts are rare anomalies often associated with intersex conditions.
    • Howard's classification categorizes these cysts based on the degree of masculinization.

    Observation:

    • Three pediatric patients presented with severe utricular and urinary infections.
    • Complications included a scrotal fistula and urinary incontinence due to intra-utricular trapping.

    Findings:

    • A perineal surgical approach was found to be difficult for cyst removal.
    • A transvesical approach provided superior surgical exposure, ease, and reconstruction.
    • The transvesical approach resulted in no significant sequelae in the treated patients.

    Implications:

    • The transvesical approach is a safer and more effective surgical option for managing complex utricular cysts.
    • Early surgical intervention may prevent severe complications like infections and incontinence.
    • Further research into the management of intersex-related anomalies is warranted.
