The liver in secondary syphilis

N M Agrawal, M Sassaris, B Brooks

    Southern Medical Journal
    |September 1, 1982
    PubMed
    Summary

Secondary syphilis can cause liver damage, mimicking other liver diseases. Prompt antibiotic treatment resolves these abnormalities, but Jarisch-Herxheimer reaction is a potential complication.

    Area of Science:

    • Hepatology
    • Infectious Diseases
    • Syphilology

    Background:

    • Secondary syphilis is a systemic infection with diverse clinical manifestations.
    • Liver involvement in syphilis is rare but can present diagnostic challenges.

    Observation:

    • Syphilitic hepatitis may mimic alcoholic liver disease or viral hepatitis.
    • Liver biopsy often reveals nonspecific reactive hepatitis, sometimes with cholestasis.
    • Jaundice may be absent despite detectable liver damage.

    Findings:

    • Liver abnormalities associated with syphilis are reversible with appropriate antibiotic therapy.
    • Penicillin treatment, while effective, carries the risk of Jarisch-Herxheimer reaction.

    Implications:

    • Syphilitic hepatitis warrants consideration in the differential diagnosis of unexplained liver disease.
    • Early recognition and treatment are crucial for managing syphilitic liver involvement.
    • Understanding potential complications like Jarisch-Herxheimer reaction is vital for patient care.
