Zinc balances in preterm infants

    Summary

Preterm infants fed medium chain triglyceride (MCT) formula achieved positive zinc balance sooner than those on bank human milk. High fat and nitrogen absorption are key for improved zinc retention in preterm infants.

    Area of Science:

    • Nutritional Science
    • Pediatric Gastroenterology

    Background:

    • Preterm infants have unique nutritional needs, including adequate zinc intake for growth and development.
    • Bank human milk and standard formulas may not always provide optimal zinc absorption for preterm infants.

    Purpose of the Study:

    • To compare zinc balance in preterm infants fed bank human milk, enriched bank human milk, or a formula with medium chain triglycerides (MCT).

    Main Methods:

    • Three groups of preterm infants were studied, receiving different milk/formula types.
    • Zinc intake, fecal excretion, and retention were measured.
    • The relationship between zinc absorption and fecal fat and nitrogen was analyzed.

    Main Results:

    • Fecal zinc excretion was high across all groups, but zinc retention was mostly negative in infants fed bank human milk.
    • Infants fed the MCT-containing formula showed mostly positive zinc balances.
    • Zinc absorption correlated significantly with fecal fat and nitrogen levels.

    Conclusions:

    • The formula with MCT facilitated earlier achievement of positive zinc balance in preterm infants compared to bank human milk.
    • Optimal fat and nitrogen absorption exceeding 90% is crucial for achieving positive zinc balance in preterm infants.
