Persistent chylothorax. Treatment by talc pleurodesis

    Background:

    • Persistent chylothorax poses a significant therapeutic challenge.
    • Conventional treatments like dietary management, thoracentesis, and tube thoracostomy drainage are often insufficient for refractory cases.

    Observation:

    • Two patients with persistent chylothorax, one with advanced lymphosarcoma and another post-thoracic aortic aneurysm resection, were evaluated.
    • Both patients exhibited resistance to standard medical and surgical interventions.

    Findings:

    • Successful treatment of chylothorax was achieved using talc pleurodesis in both patients.
    • One patient received talc powder via open thoracotomy, while the other had talc suspension instilled through a thoracostomy tube.

    Implications:

    • Talc pleurodesis is a viable and effective treatment for persistent chylothorax when other methods fail.
    • Intrapleural talc instillation, particularly through a thoracostomy tube, can be successful if the pleural space is adequately decompressed and the lung fully expanded.
