    Background:

    • Giant lymph node hyperplasia (GLNH) is typically considered a benign lymphoproliferative disorder.
    • The neoplastic potential of GLNH remains incompletely understood.

    Observation:

    • A case of chest wall GLNH presented with infiltrating plasma cell nodules.
    • Serum paraprotein of IgG lambda type was detected.
    • Immunocytochemistry revealed polyclonal plasma cells in typical GLNH areas and monoclonal plasma cells in nodular infiltrates.

    Findings:

    • The presence of monoclonal plasma cells within the GLNH lesion suggests a neoplastic process.
    • This finding indicates that GLNH can harbor or progress to a plasmacytoma.

    Implications:

    • Giant lymph node hyperplasia may have a greater neoplastic potential than previously recognized.
    • This case highlights the importance of immunophenotypic analysis in evaluating GLNH, especially when atypical cellular infiltrates are present.
    • Further research is warranted to elucidate the mechanisms underlying the transformation of GLNH into plasma cell neoplasms.
