Capitellocondylar unhinged implant arthroplasties improved elbow function and pain for rheumatoid arthritis patients. Complications included infection and subluxation, with posterolateral approach reducing ulnar nerve issues.

    Area of Science:

    • Orthopedic Surgery
    • Biomedical Engineering

    Background:

    • Elbow arthroplasty using capitellocondylar unhinged implants was evaluated for patients with rheumatoid arthritis and osteoarthritis.
    • Indications included intractable pain, instability, failed synovectomy, and limited motion.

    Observation:

    • Thirty arthroplasties were performed on 27 patients (average age 59.4) between 1976 and 1981.
    • Follow-up averaged 39.9 months.

    Findings:

    • Moderate improvements in flexion, pronation, and supination were observed, but not in extension.
    • Ewald functional scores significantly improved from 8 to 85.
    • Complications: deep wound infections (6.6%), subluxation (13.2%), and ulnar nerve paresthesia (10%).

    Implications:

    • Capitellocondylar unhinged implants can restore elbow function and alleviate pain.
    • Posterolateral surgical approach may mitigate ulnar nerve complications.
    • Further research into optimizing implant design and surgical techniques is warranted.
