    Background:

    • Suprapatellar structures are frequently missed during double contrast knee arthrography.
    • A suprapatellar plica synovialis, a remnant of embryonic development, is a recognized anatomical feature.

    Purpose of the Study:

    • To assess the incidence of suprapatellar plica synovialis during double contrast knee arthrography.
    • To differentiate incidental plica findings from symptomatic plica syndrome.

    Main Methods:

    • Review of 53 double contrast knee arthrograms.
    • Identification and documentation of suprapatellar plica synovialis.

    Main Results:

    • A suprapatellar plica synovialis was identified in 40 out of 53 (75%) arthrograms.
    • Only one patient with a plica synovialis was treated for plica syndrome, indicating most findings are incidental.

    Conclusions:

    • Suprapatellar plica synovialis is a common finding in knee arthrography.
    • It should not be misdiagnosed as an adhesion or capsular abnormality without clinical evidence of plica syndrome.
