    Summary

    Diabetic maculopathy can cause macular holes due to fibrous tissue traction. Photocoagulation may benefit patients with progressive diabetic retinopathy and poor vision.

    • Macular holes can develop in eyes with diabetic maculopathy.

    Observation:

    • Three patients with diabetic maculopathy presented with macular holes.
    • The macular holes appeared associated with fibrous proliferation on the macula.

    Findings:

    • Fibrous proliferation shrinkage or traction is a potential cause of macular holes in diabetic maculopathy.
    • Macular holes in this context may arise from mechanical forces on an edematous and exudative macula.

    Implications:

    • Photocoagulation is a recommended treatment for patients experiencing macular holes secondary to progressive diabetic retinopathy.
    • Early intervention with photocoagulation may be crucial for patients with visual acuity below 0.1.
