    Area of Science:

    • Pediatric Oncology
    • Thoracic Surgery
    • Diagnostic Imaging

    Background:

    • Intrathoracic tumors in children can present diagnostic challenges.
    • Posterior mediastinal masses require careful evaluation.
    • Lipomas are rare tumors, especially within the thorax in pediatric patients.

    Observation:

    • An 8-year-old boy presented with a large intrathoracic tumor discovered incidentally.
    • The tumor had been present asymptomatically for five years.
    • Noninvasive diagnostic techniques were insufficient to determine the tumor's nature.

    Findings:

    • Surgical intervention was necessary for diagnosis and treatment.
    • A large posterior mediastinal lipoma was successfully removed.
    • This represents the 15th reported case of an intrathoracic lipoma in a pediatric patient.

    Implications:

    • Highlights the importance of surgical exploration for persistent, undiagnosed intrathoracic masses in children.
    • Contributes to the limited literature on pediatric intrathoracic lipomas.
    • Underscores the need for considering rare diagnoses in pediatric thoracic oncology.
